Specialized Respiratory Care in Primary Care Practices

The Respiratory Care Enhancement Program was established in response to the need for specialized expertise for this population in the delivery of primary care—specifically as an attempt to pull the patient-centered neighborhood model into usual site of care locations.  By providing expertise in respiratory illness within the primary care setting, those with the possibility of an asthma or COPD diagnosis are ensured more specialized care that meets their unique care needs.  The project will achieve the following objectives: 

  1. Reduce hospitalization of asthma/COPD patients
  2. Reduce ED utilization of asthma/COPD patients
  3. Decrease or improve scores on Asthma Control Index
  4. Increase patient-reported scores on a patient empowerment scale
